Osteochondritis Dissecans of femoral head in children

Abstract
Purpose: To elucidate the causative diseases and outcomes of osteochondritis dissecans of the femoral head in children.

Materials and Methods: We reviewed the radiographs and medical records of 1242 hips of children who had diseases related to osteochondritis dissecans of the femoral head in children over a 26-year period from 1975 to 2000. They cases included idiopathic osteochondritis dissecans in one patient, Legg-Calve-Perthes; disease in 704, developmental dislocation of the hip in 418, fracture and/or dislocatiinof proximal femur in 47, and septic hip in 72.

Results: Among the 1242 hips studied, sixteen hips had osteochondritis dissecans of the femoral head. Fifteen cases were foound with the underlying disease of Legg-Calve-Perthes¡¯ disease and one case was idiopathic. They were followed for an average 12.7 years(4 to 20 years). Among the 16 cases, the outcome of osteochondritis dissecans was spontaneous healing in 11 cases, probable spontaneous healing in three, healing by drilling in one, and loose body fixed in acetabular fossa in one.

Conclusion: In the 16 cases of osteochondritis dissecans of femoral head in children examined in this study, Legg-Calve-Perthes¡¯ disease was the major underlying disease. There patients usually remain asymptomatic and may require no special treatment.
